We are seeking a knowledgeable and detail-oriented Pharmacist to join a hospital-based healthcare team. This contract role is ideal for professionals with a solid background in clinical pharmacy, medication dispensing, and patient counseling. The ideal candidate will also have experience working within government healthcare systems and be familiar with regulatory and formulary requirements in those environments. This position plays a vital role in promoting safe, accurate, and effective medication use through interdisciplinary collaboration and adherence to hospital and quality improvement standards.
Key Responsibilities:Participate in hospital-based quality improvement programs in alignment with JCAHO and HCFA standards
Interpret and verify medication orders for accuracy, completeness, and potential allergies
Ensure prescriptions align with the approved formulary or obtain necessary approvals
Identify, evaluate, and mitigate drug-drug and food-drug interactions
Utilize ScriptPro and Pyxis systems for prescription entry and processing
Prepare accurate medication labels and patient education materials
Dispense medications with attention to proper labeling and patient instructions
Provide in-depth counseling to patients regarding medication usage and safety
Clarify incomplete, incorrect, or non-formulary medication orders with prescribers
Collaborate with physicians, nurses, and other healthcare professionals to resolve medication issues
Maintain compliance with DEA regulations for controlled substances, including Schedule II documentation and recordkeeping
Verify medication orders against the Medication Administration Record (MAR)
Compound IV medications and accurately label additives as prescribed
Fill and manage unit dose carts as part of inpatient pharmacy responsibilities
Review patient charts for dosage accuracy, appropriate drug selection, and allergy checks
Adjust medication dosages based on renal function assessments
Educate and counsel patients on discharge medications for safe home use
Valid and unrestricted Pharmacist license
Minimum 1 year of experience in a hospital or government healthcare setting preferred
Proficiency with ScriptPro , Pyxis , and other pharmacy management systems
In-depth knowledge of drug interactions , renal dosing , and formulary practices
Strong communication skills for patient interaction and team collaboration
